COURSE DESCRIPTION:

This course will use sensitivity, specificity, and likelihood ratios to present an evidenced based, and clinically relevant, approach to completing an upper quarter (cervical and upper thoracic region and ribs) assessment.  The lecture and lab series includes a detailed presentation of the functional anatomy of the cervical and upper thoracic spine arthrokinematics, biomechanics, and muscle function.  Common upper quarter pathologies, including headaches, will also be covered as they pertain to etiology, patient history, and differential diagnosis.  Current literature will be incorporated to develop a screening exam for patients with headaches to determine what, if any, other medical practitioners need to be involved in the plan of care.  Finally, the participants will learn and practice, various manual therapy interventions as they pertain to the pathologies discussed in class.  The manual therapy interventions, taught in this course, include mobilizations, high velocity thrust, myofascial release, and nerve glides

LEARNING OBJECTIVES:

Upon completion of this course, the student will be able to:

Detail normal anatomical structures and arthrokinematics of occiput through T4, including the ribs.
List common differential diagnoses for musculoskeletal disorders in the suboccipital, cervical and upper thoracic spine and upper ribs regions.
Identify impairments, functional limitations, and disabilities in patients with upper quarter musculoskeletal pathologies.
Utilizing research-based evidence, discuss how to properly utilize special tests in the discrimination of upper quarter musculoskeletal lesions.
Demonstrate proficiency in the application of myofascial release and nerve flossing in the upper quarter.
Utilizing research-based evidence, demonstrate a physical/occupational therapy examination for patients with cervical, thoracic, and/or rib impairments.
Utilizing research-based evidence, demonstrate a physical/occupational therapy examination for patients with cervicogenic headaches.
Perform an examination and screen to assess for primary headache, cranial nerve disorders, and behavioral health issues.
Demonstrate graded mobilization techniques to increase ROM and/or modulate pain in the suboccicipital, cervical, upper thoracic, and rib regions.
Demonstrate a variety of high velocity thrust techniques to increase ROM in the suboccipital, cervical, upper thoracic, and rib regions.

PRESENTER: Jacob Thorp, PT, DHS, MTC

Jacob Thorp, PT, DHS, MTC received his Master of Science degree in Physical Therapy from Des Moines University, Osteopathic Medical Center in 2001. He also received his Doctor of Health Science degree with emphasis on Orthopedic Physical Therapy from the University of Indianapolis. Jacob is currently a Clinical Assistant Professor at East Carolina University in the school of physical therapy. He is also adjunct DE instructor in the school of physical therapy at the University of St. Augustine. In 2005 he became a Certified Ergonomic Assessment Specialist, CEAS from the Back School of Atlanta. In 2006 he received his Manual Therapy Certification, MTC from the University of St. Augustine. Jacob is also a practicing physical therapist; he is currently a Physical Therapist, adjunct at the Brody School of Medicine and also Clinic Administrator and Physical Therapist East Carolina University Physical Therapy. Jacob is also involved in authoring book and chapters and he also has numerous research experience.
